Online health declaration is required to access RMIT campuses during the current COVID-19 period. [Updated: 23 Sep 2021, 1PM]
The RMIT health declaration form is for the purpose of protecting the health, safety and welfare of anyone who may enter and undertake activities on RMIT campuses in Vietnam, and protecting the RMIT community from the spread of COVID-19.
All students and staff who enter our RMIT campuses in Vietnam must complete a daily health declaration form prior to coming to campus. This requirement is valid until further notice.
Step 1: Fill and submit an online health declaration form:
Step 2: Check your inbox for an email confirmation from email@example.com, which will state your access clearance status.
Step 3: Follow the instructions in the email confirmation.
Students and staff that are declined campus access will self-evaluate based on the Government’s advice and the University instructions. At risk cases will be monitored and followed up with by Student Connect (students) or Human Resources (staff).
All information collected via the health declaration form is completely confidential and only serves the prevention of COVID-19 on our campuses.
